Highland Kids Dentistry respects your privacy. We handle two kinds of information: personal information and health information. Personal information is your name, email, phone and address. Health information is medical-related and has separate legal obligations. Our use of the two are detailed below, together with your child’s Patient Rights and this website’s Terms of Use. Personal Information: we use and disclose the personal information you give us in order to contact you and perform services you request. We do not share or sell your information beyond what you approve. Period. Health Information: We use and disclose your child’s health information for treatment and payment. Treatment: We may use and disclose health information to another physician or healthcare provider who treats your child. Payment: We may use and disclose health information to obtain payment for our services. Authorization: In addition, you may give us written authorization for other purposes, and you may revoke your authorization in writing at any time. Without such authorization, we cannot use or disclose your health information for any reason except as described below. Disclosure: We must disclose your child’s health information to you, as described in Patient Rights below. We may disclose it to a family member, friend or other person to the extent necessary to help with your child’s healthcare or with payment, but only if you agree that we may do so. With your permission, we may disclose it to another family member or other person responsible for your child’s care. Emergency: In the event of your incapacity or in emergency circumstances, we may disclose your child’s health information that is relevant to the recipient’s involvement in your child’s care. Marketing: We will not use health information for marketing without your written authorization. Reminders: We may use or disclose health information to provide you with appointment reminders such as voicemail messages, postcards or letters. Access: As legal guardian of your child, you have the right to review or receive copies of health information, with limited exceptions. You must make a request in writing (email is acceptable) to obtain access to this health information. You may request that we provide copies in a hard-copy format, and we will use the format you request unless we cannot practically do so. Alternative Communication: You have the right to request that we communicate regarding health information by alternative means or to alternative locations. You must make your request via email or in writing, and you must specify the alternative means or locations. Amended Records: You have the right to request that we amend your child’s health information. Your must make your request via email or in writing, explaining why the information should be amended. We may deny your request under certain circumstances. Disclosure Accounting: You have the right to receive a list of instances in which we or our business associates disclosed your child’s health information for purposes other than treatment, payment, or other activities for the last six years. If you request this more than once in a 12-month period, we may charge you a reasonable, cost-based fee. Electronic Notice: You are entitled to receive this Notice in written form, at your verbal request or via email or in writing. Restriction: You have the right to request that we place additional restrictions on our use or disclosure of your child’s health information. We are not required to agree to these additional restrictions, but if we do, we will abide by our agreement except in an emergency. Terms of Use: Everything in this website is covered by these Terms of Use, and by your presence, you agree to be bound by these terms. Ownership: This website is owned and maintained by Highlands Kids Dentistry (“HKD”, “us” or “we”). We retain all rights, including copyrights, to all website pages. Any failure to enforce any of these terms is not a waiver of our rights, and these terms are deemed in full effect regardless of whether we enforce them. Accuracy: Despite our good-faith effort to present accurate information, errors may occur.
